Liability Waiver

The Bay Basketball Association, Inc. (BBA) is organized for the enjoyment, development, and benefit of team members. Coaches and volunteer staff give generously of their time to this end. The player and his/her parent(s) or guardian(s) choose to be involved with all related activities of the BBA (open gyms, clinics, try-outs, practices, exhibitions, games, tournaments, etc) at their own risk.

Coaches, staff, the BBA, its board and officers, the City of Bay Village and its recreation department, Bay Schools, other schools/locations hosting BBA events and other partners of the BBA are not liable for injuries incurred within the City of Bay Village or at a location outside of the City of Bay Village when such injuries occur as a result of participation in a BBA related event.

Player registration must be completed and submitted prior to participating in any BBA travel team activity (open gym/instructional time, try-out, practice, etc). The undersigned must have legal custody of the player. Only one legal parent (custodial parent)/guardian needs to sign this release, though is understood to be signing for any other parent/guardian.

By registering you understand and agree to the above paragraphs in this player release form as a condition for participating in the activities of the Bay Basketball Association, Inc.
